Immigration Medical Exams
As part of the immigration process for obtaining lawful permanent residency (green card) or certain entry permissions into the United States, U.S. Citizenship and Immigration Services (USCIS) requires a medical examination.
Key Requirements:

Most applicants filing for adjustment of status (Form I-485) must undergo an immigration medical examination performed by a USCIS-approved physician.

The results are documented on Form I-693, Report of Immigration Medical Examination and Vaccination Record.

This examination helps confirm that the applicant is not inadmissible on public health grounds, including required vaccinations against vaccine-preventable diseases.

Your Appointment Visit
Before Your Visit
Please prepare and bring these documents:

Valid, unexpired passport or government-issued photo ID

Any available medical records

Vaccination/immunization records
Additional tips:

Arrive 15 minutes early to complete required paperwork.

Wear comfortable clothing for the physical exam (short sleeves for blood draw).

If under 18, a parent or legal guardian must accompany you.
At Your Visit

Tuberculosis (TB): Blood test required for ages 2+

Syphilis: Blood test required for ages 18–44

Gonorrhea: Urine test required for ages 15–24

Hansen’s Disease (Leprosy): Clinical skin exam and history review for all ages

Vaccinations: Assessment and required shots

Physical & Mental Exam: Full evaluation, including history, for signs of harmful disorders or drug abuse, all ages.
After Your Visit
Once your lab results are available, our staff will contact you to:

Review your test results with you.

Inform you that your forms are ready for pickup.
You will receive:

A sealed envelope containing the original completed Form I-693 (required for submission to USCIS – do not open).

A copy of Form I-693 and your lab results for your personal records.
